Stretcher on a commercial flight: when it works and how it is arranged

A stretcher on a commercial flight lets a patient who has to lie flat travel on a normal scheduled flight, with a doctor or nurse at their side. For a stable patient it is often the most practical middle way between a regular seat and a dedicated air ambulance. It is only possible with the airline's approval, and it takes careful planning.

Who a stretcher flight is for

A stretcher on a scheduled flight usually fits a patient who:

  • is medically stable, with no need for intensive care during the flight;
  • cannot sit upright for take-off, landing and the flight itself, for example after a fracture or surgery;
  • needs a medical professional nearby for medication, pain relief or observation.

It does not fit a patient who needs ventilation, continuous intensive monitoring or rapid intervention. For those patients, an air ambulance is the safer choice.

Airline approval: the MEDIF

Airlines decide themselves whether they carry a stretcher patient. The usual route is the MEDIF (medical information form): the treating doctor completes it with the diagnosis, current condition, the need for a stretcher, oxygen or other equipment, and whether a medical escort travels along. The airline's medical department reviews it and approves, declines or sets conditions. Not every airline or route offers stretchers, so we only confirm the plan once that approval is in.

The medical escort

A stretcher patient travels with a qualified doctor or nurse, chosen to match the medical needs. The escort meets the patient at the hospital, stays with them through the airport and the flight, manages medication and comfort, and hands them over at the destination with a report. Airlines often make an escort a condition of carriage.

How it is arranged, step by step

  1. We receive the medical report and discuss the case with the treating doctor.
  2. The doctors confirm that a stretcher on a scheduled flight is safe.
  3. We identify a suitable flight and submit the MEDIF to the airline.
  4. You receive a written quote before anything is booked.
  5. After airline approval: ground ambulance to the airport, boarding with the escort, the flight, and a ground ambulance on arrival.
  6. Handover at the receiving hospital or home. See bed-to-bed transfer.

Stretcher flight or air ambulance?

A stretcher on a scheduled flight depends on airline schedules and approval, and the patient travels in the cabin of a normal aircraft. An air ambulance is a dedicated aircraft equipped for intensive care, flown by a licensed operator, with departure planned around the patient. The doctors decide which one is safe; cost and timing are discussed openly in the written quote. We coordinate both and do not own aircraft.

Frequently asked questions

Can any patient travel on a stretcher on a commercial flight?

No. It is for medically stable patients who need to lie flat but do not need intensive care during the flight. The airline must approve each case.

What is a MEDIF?

A medical information form used by airlines. The treating doctor completes it, and the airline's medical department decides whether and on what conditions the patient can fly.

Does a doctor or nurse have to travel with a stretcher patient?

In practice yes: a qualified escort travels with the patient, and airlines often require it as a condition of carriage.

Is every airline able to carry a stretcher?

No. Not every airline or route offers stretchers. We plan the flight only after the airline has approved the case.

Is a stretcher flight cheaper than an air ambulance?

Often, but it must be medically suitable. You receive a written quote for the options before anything is booked.

Can a family member travel on the same flight?

Usually a family member can book a regular seat on the same scheduled flight.
